• I have a problem with my lungs, breathing, heart and/or blood that affects my normal physical or mental activities and I am currently receiving ongoing medical attention.
• I have had surgery within the past 12 months, or am currently seeing a doctor because I have ongoing problems related to a previous surgery.
• I have had blackouts, migraines, seizures, strokes, serious head injuries, or a persistent neurological injury or condition.
• I am currently receiving treatment (or have needed treatment within the past 5 years) for psychological problems, personality disorder, panic attacks, or drug or alcohol addiction; or You have been diagnosed with a learning disability or developmental disability and are currently receiving medical care.
• I currently continue to see a doctor for back/lower back problems, a hernia, and an ulcer.
• I am currently continuing to see a doctor for a recent gastrointestinal illness, including diarrhea.
• I am taking prescription medications (except oral contraceptives or antimalarials other than mefloquine/Lariam).
